Fix: CSS key extraction, hopefully fixes not correctly
retrieved CSS keys with some titles and drives, causing
"Clone 1" errors with CloneDVD.
Fix: Arccos protection was not removed, if protected title
only had one single chapter, e.g. "Layer Cake, Disc 2" (UK)
Some minor fixes and improvements
